Please use the space below to communicate in detail two or three significant events...special moments etc. between yourself and the person(s) the poem is being written for. You need to define their relationship to you…be it a spouse, fiancé, relative, friend, pastor, co-worker, boss, secretary etc., as well as share any love, adoration, admiration, care or gratitude etc. you may have for them. Finally, take a moment to write down between 3-5 adjectives that describe the person(s) the poem is being written for, while giving an example of something they may have done or said etc. that causes you to describe them in that particular manner. For ex; Kind, lovely, giving etc. These will be adjectives used in some way in the personalized poem. Be sure to provide the particular scripture or quote that you desire to be part of the plaque. *If for a wedding ceremony please include the date of the wedding and full name of bride & groom.
